Welcome! Please fill out and print the forms below and bring them with you to your first appointment. If you'd rather complete them in the office, plan to arrive 15 minutes before your scheduled appointment time.

Don't have a printer? No problem — we'll have all forms available at the office. Just call ahead so we know to set them aside for you.

What Else to Bring

Beyond the Forms

• A list of current medications (or the bottles).
• Any recent X-rays, treatment plans, or photos from prior dentists.
• Your dental insurance card (and medical insurance if relevant).
• A list of questions or concerns — the consultation works best when you've thought about what you want to address.
• Your referral letter if you've been referred by another doctor.
